What happened
Bard Access Systems recalled four catalogue numbers of its Needle Kit for Powered Driver — needle sets for the BD intraosseous vascular access system, lots BSLC9465 through BSLC12123 — on July 30, 2026: manufacturing dimensions out of tolerance can make the obturator hard to remove after placement.
The obturator can rotate and lock at the needle-hub interface during insertion, or with improper removal technique. Users complained; bench testing and clinical observation confirmed it. Intraosseous access is an emergency route when veins cannot be reached — the kit's stiffest moment is the one the defect picks.
